The below is a transcribed copy of a blank Department of State (DoS) form that was faxed to me.

Your applcation for a U.S. passport is being delayed issuance ponding the receipt of additional evidence of identity and/or citizenship.

This information is being requested under the authority of Volume 22 of the Code of Federal Regulations, Seciton 51.40, which states that the applicant has the burden of proving that he or she is a citizen of the U.S. and /or 51.28 concerning identity. Towards this end, under 51.54 and/or 51.28, we may require an applicant to submit other evidence deemed necessary in order to establish his or her citizenship and/or identity.

Therefore, please complete the attached questionnaire concerning your family, (including their dates of birth, places of birth and whether U.S. passports have been issued to them, giving the approximate year and number, if known), residences (including time living outside the (U.S.), references, schools and employment. Submit the completed questionnaire and supporting documents from the list provided to:

22 CFR 51.40 Burden of Proof
The applicant has the burden of proving that he or she and any persons to be included in the passport are nationals of the United States.

22 CFR 51.54 Requirement of Additional Evidence of US Citizenship
Nothing contanied in paragraphs 51.43 through 51.53 shall prohibit the Department from requiring an applicant to submit other evidence deemed necessary to establish his or her US Citizenship.

22 CFR 51.28(c) Identity of Applicant
Ay official receiving an application for a passport or any Passport Issuing Office may require such additional evidence of identity as may be deemed necessary.

22 CFR 51.55 Return or Retention of Evidence of Citizenship
The passport issuing office will generally return to the applicant evidence submitted in connection with an application for passport facilities. However, the passport issuing office may retain evidence when it deems necessary.
